Before your appointment

If you plan to have insurance pay for your evaluation, therapy services, or device, please be sure to have a physician referral sent to MSHC before you make your appointment.
Before you arrive, please download and complete these forms. Don't forget to bring them with you to your appointment.

Upon Arrival

If you printed and filled out the MSHC forms OR if you received forms in the mail, please complete and bring them with you to your appointment.

Bring

• MSHC forms
• driver's license/picture ID
• insurance card
• co-pay (if applicable)

Payment options

• We accept most major insurance plans, Medicare, Medicaid, and we offer a self-pay option.
• Client Assistance Program (CAP)
• Pay your bill
