Connect with DRS
Connecting with DRS is simple. Just follow the process below:
- Complete the relevant DRS Student Introduction Form. There are multiple forms to choose from, including Permanent Conditions Student Introduction Form, Temporary Conditions Student Introduction Form, Pregnant Students Introduction Form, Housing Accommodation, Disability-Related Housing Accommodation Requests Only, Dining Accommodation Requests Only, Totally and Permanently Disabled Waiver Application. Choose the form that fits your circumstance. For example, if you plan to request academic accommodations, housing accommodations, dining accommodations and the Totally and Permanently Disabled Waiver, you should complete the Permanent Conditions Student Introduction Form, and, if you only plan to request an emotional support animal (ESA), you should complete the Disability-Related Housing Accommodation Requests Only Form. Once you complete and submit the relevant form, you will be assigned a coordinator who will contact you via email to schedule your initial appointment. Please note that students who are only requesting the Totally and Permanently Disabled (TPD) Waiver only need to complete step 1 and 3.
- Complete your initial appointment. During the initial appointment, you will discuss accommodations and services with your coordinator.
- Submit documentation. Documentation can be submitted through DRS online when you submit the Student Introduction form. It can also be faxed to 901-678-3070, emailed to drs@memphis.edu, or hand-delivered to 110 Wilder Tower. If you do not have documentation that is readily available, please go ahead and complete the first two steps in the process, so that we can determine what documents are needed and connect you with resources for obtaining documentation. If you are only requesting the TPD Waiver, you must submit either the TPD Waiver Application or current proof of eligibility for SSI/SSDI.
Future students should wait until they have been accepted at the UofM before they complete this process.
Connecting with DRS initiates the accommodations process. There may be additional steps to take to determine the appropriate accommodations for your classes each semester based on the nature of those classes and your individual circumstances. Additionally, accommodations are not retroactive, so it is important that you initiate and complete the accommodations process as soon as you determine accommodations may be appropriate.
If you have questions regarding this process, please contact DRS at 901-678-2880 or drs@memphis.edu.
