Documenting Your Condition
DRS recognizes that barriers to obtaining documentation of a diagnosis may exist and can create hardship for students with regard to presenting documentation from a health care professional. We encourage students to submit documentation that verifies the diagnosis and/or history of services (most recent IEP, 504 plan, psychological testing report, etc.) and is readily available along with the Student Introduction Form. If such documents are not readily available or there is no prior diagnosis, determinations on individual documentation needs will be made during the initial appointment. You may also choose the relevant form below to have your healthcare professional complete:
- Medical Documentation Form: This form should be used by students who have permanent conditions, temporary conditions, who are pregnant, or students requesting dining accommodations.
- Housing Accommodation Documentation Request Form: This form should be used by students who are requesting any accommodations in residential setting, including a request for emotional support animals.
- Totally and Permanently Disabled Waiver Form: This form should be used by students who are requesting the Totally and Permanently Disabled Waiver.
It should be noted that documentation policies differ from one institution to another. Disability Resources for Students documentation policy is designed to fit the University of Memphis campus climate and does not consider documentation requirements external to the UofM. If a student will attend another institution after attending the UofM or take a standardized test (GRE, MCAT, etc.) administered by an outside agency, the student is responsible for researching those documentation policies and should investigate requirements at least six months prior to an anticipated start date or test date.
