Church Health Center
To become an established patient of Church Health, you must be an uninsured resident
of Shelby County and meet one of the following requirements:
- Work 20 hours or more per week,
- Assigned to Church Health through your insurance carrier or MEMPHIS Plan,
- A full–time student (12 or more credits), or,
- The primary caregiver of children in the household
