Name Change on Official University Record

  • Use this form to change your name on your records at the University. Use it regardless of your Relationship to the University--Student, Employee (Administrator, Faculty, or Staff), Alumnus or Alumna, Friend, or Donor.
  • Check ALL Relationships that apply. Sign the form. Send the completed form and any required documentation to the office in the first Relationship checked.

Advising/Planning Registration Worksheet

  • Use this form to plan your semester class schedule. Include desired classes and alternatives.
  • The form is maintained by the Academic Counseling Center. The link and information are provided here for your convenience.

Audit - Request for Permission to Audit Class

  • Use this form to request permission to audit a course.
  • Instructions accompany this form; be sure to read them as well as bulleted information at the top of the form.

Consent to Release Your Education Record

  • DO NOT USE THIS FORM TO REQUEST AN OFFICIAL TRANSCRIPT. (Use an Official Transcript request form instead.)
  • Use this form to authorize the release of your education record (ex., grades) to another person (ex., a parent or guardian) other than the University of Memphis and its employees.
  • See Student Consent to Release Education Records for instructions and related information.

Credit/No Credit - Request Change to/from Credit/No Credit (web)

  • Use this form to request permission to change from standard grading to credit/no credit grading, or from credit/no credit grading to standard grading.
  • By academic policy, you may not submit this type of request after the last day to add classes for the part of term has passed.
  • This request is allowed for undergraduate courses only. Instructions accompany the form; be sure to read them as well as bulleted information at the top of the form.

Credit by Examination - Graduate

  • Use the form to apply for GRADUATE credit by examination.
  • The form is maintained by the Graduate School. Links and information are provided here for your convenience.
  • Refer to the Credit by Exam and Course Validation sections of the online Graduate Catalog for guidelines and instructions pertaining to Graduate Credit by Examination. Contact the Graduate School if you have further questions.

Credit by Examination - Undergraduate

  • This is a hard copy form and is available only in the Registrar's Office.
  • Use the form to apply for UNDERGRADUATE credit by examination.
  • See Credit by Exam for complete information and instructions.

Excess Transcripts Request - Off-Campus Payment Form

  • Do not use this form if you intend to make your request at the Registrar's Office (003 Wilder Tower). A different payment form is available for you there.
  • Use this form if you have submitted an off-campus request for transcripts and you have requested more than 20 transcripts. You will be charged $5.00 for each transcript over 20 copies. Use this form to make your payment.

FERPA Form for non-Student System Users

  • Use this form if you are a student and must view or access student data to perform your University duties (ex., duties expected of a student worker or research assistant).
  • Complete the form, obtain all required signatures, and submit the original to the Registrar's Office, 003 Wilder Tower. Retain a copy of the completed form in your department's files.

FERPA Student Researcher Agreement

  • Use this form if you are a student who is requesting access to confidential student records in order to conduct a University-approved research project.
  • The Institutional Review Board (IRB) must sanction your project and the use of the data involved. Submit the completed agreement to Curriculum and Assessment, 217 Administration Building.

Full-Time, Online Only Student Immunization Exemption Request

  • Use this form if you are a full-time, online-only student who wants to be exempted from MMR and Varicella (chicken pox) immunization requirements.
  • Refer to Student Health Services' website for immunization information.

Letter of Recommendation

  • Use this form to request a letter of recommendation from a faculty member.

Official Transcript - Off-Campus Request for Official Transcript

  • Use this form to mail or fax your request for an OFFICIAL transcript from the U of M.
  • Do not use this form if you intend to make your request at the Registrar's Office (003 Wilder Tower). A different form is available for you there.

Request for Student's Taxpayer Identification Number & Certification
(U of M W-9S)

  • Use this form to submit your social security number (SSN) or individual taxpayer identification number (ITIN) to the U of M so that the University can file a
    1098-T (Tuition Statement) form with the IRS and furnish such a statement to you.
  • Instructions are on the form.
  • Deliver or mail the completed form to Registrar's Office or the Finance Office, or FAX it to 901-678-2772. (See Part III of the form for address and FAX information.) *DO NOT E-MAIL THE FORM.*

Restrict the Release of Directory Information

  • Use this form to prevent your Directory Information from being published in the Campus Directory.
  • Check ALL "acknowledgment" statements. Sign the form. See Release of Student Education Records (What Is Directory Information?) for instructions and related information.

Veterans Advance Pay Request - U of M

  • Use this form to apply for advance payment. Complete and sign the form, then deliver it to Veterans Educational Benefits & Certification, 003 Wilder Tower, Memphis, TN 38152-3520. FAX: 901.678.1425.

Veterans Request for Certification

  • This form is web-based and requires that you have a U of M Username and Password.
  • Use this form to register with the U of M's Office of Veterans Educational Benefits & Certification every semester in which you take classes.

VA Education Forms

Yellow Ribbon Request

  • Use this form when registering for classes under Chapter 33, the Post-9/11 GI BillĀ® Yellow Ribbon Program.
    (GI BillĀ® is a registered trademark of the U.S. Department of Veterans Affairs (VA). More information is available at the official U.S. government Web site at www.benefits.va.gov/gibill.)

Faculty & Staff: Access & Authorized Signatures

Access to Banner Student System

  • Use this form to request access to the Banner Student System.
  • Instructions accompany the form.

Authorized Signatures: Dean's Signature

  • Used by the Provost's Office to name a Dean and submit the Dean's handwritten signature to the Registrar's Office. The Dean's signature authority is detailed on the form.

Authorized Signatures: Dean's Designee's Signature

  • Used by the Dean to identify his or her official Designee and to submit the Designee's handwritten signature to the Registrar's Office. The Designee's signature authority is detailed on the form.

Authorized Signatures: Department Chair/Head's Signature

  • Used by the Dean or Dean's Designee to identify a department's Chair or Head and to submit that individual's handwritten signature to the Registrar's Office. The Chair/Head's signature authority is detailed on the form.

Authorized Signatures: Add Authorized Signature

  • Used by the Dean or Dean's Designee to (1) assign signature authority for enrollment exceptions to specific individuals within the College or School, and (2) submit the individuals' handwritten signatures to the Registrar's Office.
  • Available enrollment exceptions are listed on the form. See Authorized Signatures for links to lists of current officials and their authority.

Authorized Signatures: Delete Authorized Signature

  • Used by the Dean or Dean's Designee to remove officials who no longer have signature authority for the College or School.
  • Processing instructions are on the form. See Authorized Signatures for links to lists of current officials and their authority.

  • Use the forms to request course revisions, new courses, or special topics courses.

FedEx Institute of Technology Room Request

  • *FOR USE BY THE FedEx INSTITUTE OF TECHNOLOGY ONLY.*
  • Use this form to reserve rooms for an upcoming term.

Instructor Assignment Correction Log

  • Use this form to revise the instructor of record for a course/section.

Non-Standard Course Offering

  • Use this form to schedule courses taught at non-standard times, contract courses (University funds <50%), or sponsored courses.

Section Maintenance

  • Use this form to cancel, add, modify, or reactivate course sections.
  • Refer to the instructions on the form.

Section Maintenance (Word)

  • Use this MS-Word version of the form only if you want to e-mail Section Maintenance requests:
    1. Complete the form by highlighting each appropriate blank field and replacing it with data.
    2. Save the completed form to your system with a unique name.
    3. Login to your official U of M e-mail account: your request must come from this account.
    4. E-mail the file (saved request) as an attachment to:

Academic Program Maintenance

  • Use this form for ALL academic program changes, including college, degree, major(s), minor(s), concentration(s), expected graduation term, catalog term, or student level (attribute).
  • Instructions are available with the form.

(Request Access to Student's) Academic Record/Confidential File

  • Use this form to request access to student data for legitimate business purposes.
  • Instructions are available with the form. See Faculty and Staff Access to Student Records for instructions and related information.

Change in Reported Never-Attended or Stopped-Attending Date

  • Use this form to change one of these attendance dates for a student if the online grading period has ended. If the grading period is still open, change the date online instead.
  • Refer to the instructions on the form.

Course Overload Approval

  • Use this form to approve either undergraduate or graduate course loads.
  • Instructions are available with the form.

Degree Certification Correction

  • Use this form to correct information pertaining to a previously awarded degree.
  • Refer to the instructions on the form.

Degree Certification Exception

  • *USED PRIMARILY BY GRADUATION ANALYSTS.*
  • Use this form to request an exception to degree certification.
  • Refer to the instructions on the form.

Grade Change

  • Use this form to change a student's grade after the online grading period has ended. If the grading period is still open, change the grade online instead.
  • Pay particular attention to the instructions in the "Signature Stamps Not Allowed" section.

Grade Statement

  • Use this form to assign a grade to a student who attended your class but whose name did not appear on your class list by the grading period.
  • Refer to the instructions on the form.

Incomplete Grade Extension

  • Use this form to extend an "I" grade for a student.
  • Refer to the instructions on the form.

Instructor Sign Off for Late Registrations/Late Adds

  • Use this form to approve late registrations or late adds after the official College Approval End Date has passed.
  • If the student is requesting a Late Registration, he or she must submit an approved Request to Register After Late Registration form to the Registrar as well.

Letter of Recommendation

  • If a student requests a letter of recommendation, have him or her complete this form.
  • Because some part of a student's education record is likely to be involved, a letter of recommendation for a student can have FERPA implications. Be sure to read Letters of Recommendation.

Request for Approval to Register After Late Registration

  • Use this form to approve a student's request to register for classes after the last day of Late Registration has passed.

Schedule Adjustment

  • *FOR ADVISOR USE ONLY.*
  • Use this form to initiate all schedule adjustments--adds, drops, and withdrawals.
  • Instructions are available with the form.

Request for Disclosure of Education Record Information and Certification of Dependent Status

  • Use this form to request access to your student's academic record; your student must be a dependent for federal income tax purposes.
